Demographic Survey

In an effort to better assist the communities we service, and in support of the Idaho Department of Health and Welfare, the Idaho Child Welfare Research and Training Center, Casey Family Programs, and Boise State University's commitment to the infusion of diversity, we are teaming together to build a data base that will better reflect accurate demographics throughout the State of Idaho.  In an effort to reflect all groups we are asking that you please share with us the information listed as Optional on the registration form, which will be recorded in our state-wide data base.  Please note participation in the demographic survey is optional, and that by completing it you are giving us permission to store and use your information for networking purposes.
